By signing below, I/we confirm that I have read and understand the following:

Club Fees: The following fees are required for participation in FC Vegas Academy:

Monthly Club Fee: Competitive Ages u19-u11 at $100/month
Academy Ages u10-u4 at $70/month
Competitive Gear Set: $400 (one-time)
Academy Gear Set: $150 (one-time)

Payment Responsibility: I understand that fees are NOT collected through this form. All payments must be made separately through the FC Vegas Academy Club Fee page on this website.

Automatic Monthly Billing: By submitting payment for the first monthly club fee of $100, I authorize FC Vegas Academy to automatically charge the same payment method every month on the same date going forward. This will continue until I formally request cancellation with the club.

Additional Expenses: Fees related to Tournaments, Traveling, Leagues, Camps, and any other academy activities are separate from the monthly club fee and are NOT included in the $100/month. Families will be notified of these costs as soon as they are available so that proper arrangements can be made in advance.

Cancellation: To stop automatic billing, I must notify FC Vegas Academy in advance. Cancellations made after the billing date will take effect the following month. Monthly fees already charged are non-refundable.

Important: Registration is not considered complete until all applicable fees have been paid. Failure to submit payment may result in limited participation.

I, the undersigned player/guardian, hereby acknowledge the inherent risks associated with participation in elite soccer training and competition. By completing this form, I release FC Vegas Academy from all liability related to injuries sustained during academy activities, including tryouts and formal training sessions.
